The post sits within Young People's Services Youth Homelessness Prevention Team. Your role will be two fold. You will support 16 and 17 year old young people who present as homeless as part of a multi-disciplinary team. The team works in partnership with the District and Borough Councils housing teams, social workers and supported housing providers. The services goal is to enable the young person and their family to rebuild their relationship into one that is positive, constructive and enables their exclusion from home not to happen.

Within your role you will work with the young person in a holistic and solution focused way that best enables them to develop in all areas of their lives. You will support and undertake Joint Assessments of the families’ needs with your housing and social care colleagues. You will need to be skilled at multi-agency working and developing clear and positive working relationships with a wide range of agencies.

The main body of your working week will be in the north of West Sussex covering Crawley, Horsham and Mid-Sussex Districts. However you will be required to provide cover for other Personal Advisors in all other parts of West Sussex.

The second part of your role will be to act as the Personal Advisor (as defined in the Leaving Care 2000) for 18 to 25 year old care leavers. You will be responsible for coordinating the support and Pathway Plan for these young people.
