Take Home and Settle enables vulnerable people, whose hospital discharge is delayed due to a lack of support, to return home safely and be settled comfortably. Relative Support will provides families with good quality information and advice to support them to make life changing decisions about their loved ones housing and care options. As the service is delivered in 7 hour shifts (less ½ hour break) 7 days a week, a rota will operate to ensure weekend working is shared. We have some permanent shifts and require bank staff to cover holiday and sickness absence. You need to have a full driving licence and use of a car.
